At our LIFE at Home Day Health Center*:

- Transportation to and from the Center
In your Home*:
- Personal care
- Homemaker/chore services
- Home-delivered meals
- Adaptation of the home for disability

- X-rays
- Dentistry
- Optometry and eyeglasses
- Foot care

And our staff is on call 24 hours a day, 7 days a week, including holidays. Call us anytime at (888) 843-0443 and we can help. Members must receive all health care services including hospital, physician, or nursing home (except for emergency services) through LIFE at Home.
* All services must be approved by the LIFE at Home team and agreed to by the LIFE At Home member. LIFE at Home members may be fully and personally liable for the cost of unauthorized or out of the LIFE at Home network services.
