What is LIFE at Home?
LIFE at Home is a medical care and personal support plan that aims to keep you active, healthy, and living where you prefer – in your own home.
Who provides the care at LIFE at Home?
At the heart of LIFE at Home are caring staff who work with you to develop a personalized care plan. Our top-notch professionals -- doctors, nurses, physical, occupational, and recreation therapists, social workers, home health aides, dietitians, home health aides, and many more – will provide most of your care. As needed, we also arrange for other services such as laboratory, x-ray, medical specialists, hospital and nursing home care.
Besides medical care, what else does LIFE at Home offer?
LIFE at Home can provide just about any personal care or other help needed to keep you active and healthy in your home – it’s all based on your personalized care plan. And, LIFE at Home can provide support and respite for your family.
What does LIFE at Home cost?
If you are on (or eligible for) Medical Assistance and living in the community there is no cost to you. If you are on Medicare but not Medical Assistance, you will have a monthly premium payment. Members who are ineligible for Medicare or Medicaid may choose to pay privately. A LIFE at Home representative can help you find out if you are eligible.
Where is LIFE at Home located?
A LIFE at Home Center is located at 701 Chester Pike, Sharon Hill, PA. Services are provided primarily at the LIFE at Home Center, or at home as needed.
Does anyone live at the LIFE at Home Center?
No. Our members live in their own homes or with their families.
How would I get to the LIFE at Home Center and what would I do there?
Transportation is provided. One of our specially equipped vans will pick you up at home. At the Center, you may see the nurse or doctor for a check-up, get physical or occupational therapy, or meet with your social worker depending on your needs. You’ll get a hot lunch, plus take part in social and recreational activities with other members at the Center.
What if I have a problem when the Center is closed?
The LIFE at Home team is on call 24 hours a day, 7 days a week, including holidays.
What if I need hospitalization or 24-hour skilled nursing care?
The LIFE at Home team will arrange your admission, and we will manage your care and follow-up in the hospital or nursing home.
Can I continue to see my family doctor?
The LIFE at Home program includes an on-site medical doctor at the Center, which has a state-of-the-art medical clinic. If you join LIFE at Home, you are asked to change your primary care physician to our on-site medical staff. Our doctors have specific training and experience dealing with the problems facing seniors, such as diabetes, high blood pressure, and Alzheimer’s. LIFE at Home is different from most health plans because it is all-inclusive care. Besides the doctor, your healthcare team includes clinic and home care nurses, social worker, physical therapist, occupational therapist, and dietitian. You must receive all of your health care services through LIFE at Home. LIFE at Home members may be fully and personally liable for the cost of unauthorized or out of the LIFE at Home network services.
What if I don’t like group activities or my health limits me from coming in - do I have to come to the Center every day?
It’s a matter of individual needs. Members generally come to the Center 2 or 3 times a week. Some come every day while others are only there a few times a month. If needed, LIFE at Home will provide services in your home or at a local adult day center if you don’t live near the LIFE at Home Center.
What if I need to see a specialist?
If you need to see a specialist, such as a cardiologist, LIFE at Home will make arrangements and even provide transportation, if necessary.
Who can join LIFE at Home?
LIFE at Home may be for you if you are:
- Are age 55 or older
- Have health problems that make it hard to live at home
- Live in Chester or Delaware Counties, PA
What if I change my mind, can I quit LIFE at Home if I want?
LIFE at Home is a program of choice, and you may disenroll at any time.
